
Registration InformationWoman's Hospital offers two convenient ways for you to preregister for maternity and surgical services. You may preregister online or complete the preregistration form provided by your doctor and send it to us at: Woman's Hospital P.O. You will need to sign your paperwork and submit any deposit/payments necessary according to your insurance plan during your OB Express registration (45 days prior to delivery) or surgery admission. Maternity Patients We call our process for registering maternity patients "OB Express." It includes preregistration and a visit to the hospital forty-five (45) days before your due date to sign consent forms. Please bring your driver's license and insurance card. In addition, please be prepared to pay any co-payment or deductible that is due prior to admission. If you are unable to pay this portion at the time of service, you will be expected to contact the financial counselor to make financial arrangements. Payment may be made by cash, check or credit card. Providing this information in advance helps us provide you with the best possible service. When you need to be admitted to the hospital, you will not have to wait in the lobby to sign forms and take care of paperwork while possibly in labor and uncomfortable. During your preadmission visit, the representative in the Admissions Department will also give you additional written instructions on what to do when you are ready to come to the hospital. The Admissions Department is located at entrance 4. We encourage all maternity patients to preregister. If for any reason you are unable to stop by our registration area to complete your OB Express registration prior to your due date, you can complete your registration by phone by contacting our Patient Access Coordinator Monday through Friday from 9:00 AM – 5:30 PM at 225-924-8752. If you choose to register by phone, you will need to sign admission forms and pay any co-pay or deductible that is due when you arrive at the hospital in labor. If you do not complete your registration before your labor starts, you will need to sign admission forms when you arrive at the hospital in labor. Call 225-924-8117 if you have any questions about registration. It is very important that you complete these forms. If you are admitted to the hospital and then sent home because of false labor, you need to stop in the Admissions Department on your way out to sign your consent forms again. Surgery Patients Your physician’s office will schedule an appointment for you with the Woman's Hospital Preadmissions Center. Your admission paperwork is done at this time, including signing consent forms. Consent forms are legal documents and are required for each admission to the hospital. Please bring your insurance card and driver’s license. It is very important to keep this scheduled appointment. Please be prepared to pay your co-pay and deductible at this time. You will need about one hour for this appointment. We suggest that you do not bring young children with you to this appointment. It may cause delays for you and other clients. We accept Visa, MasterCard, and Discover cards. If payment in full is not possible, the Collections office must be notified and arrangements made. Failure to set up and keep acceptable payment arrangements can result in collection agency placement.
